2. Scope of Services
Complete Direct Primary Care provides:
Direct Primary Care (DPC) services through a membership-based model
The Website is intended for informational and administrative purposes only. Use of the Website does not establish a physician–patient relationship unless and until you formally enroll in services and complete required intake and consent processes.
3. No Emergency Care
This practice does not provide emergency or urgent care services.
If you are experiencing a medical emergency or life-threatening condition, call 911 or go to the nearest emergency department immediately.
4. Medical Disclaimer
Medical information provided on this Website is general in nature and is not a substitute for individualized medical evaluation, diagnosis, or treatment.
All medical services are provided at the sole discretion of a licensed physician based on clinical judgment, available information, and applicable laws and standards of care.
5. Eligibility
Direct Primary Care membership is available to adults and children as outlined on the Website. Pediatric membership requires an enrolled adult parent or guardian.
Patients who are Medicare beneficiaries are not eligible for Direct Primary Care membership but may receive wound care services when medically appropriate and billed separately.
6. Payments, Membership, and Refunds
Direct Primary Care services are provided through a monthly membership fee
Wound care services are not included in the Direct Primary Care membership
The practice does not submit insurance claims for Direct Primary Care services
Refunds, cancellations, and membership terms are governed by the Refund & Cancellation Policy, which is incorporated into these Terms by reference.
